Where does noise come from in my workflow?

I sometimes have problems with noisy images, and I can't quite get to the bottom of it. So, this is what I might do with a typical picture...where might noise be creeping in...where might I be better...

Yes, I have it now. It's sensor size. I have 25mm showing on a sensor that is (apparently 22.2mm wide) which gives me 22.2/25 = 0.9 (nearly) to 1.
