I've used jalbum
www.jalbum.net in the past. This is free and very easy to use.
These days, I tend to use the built-in web album generator in Capture One, I think there's a similar "create web page" feature in the later versions of PS Elements, but this is from memory.
For uploading onto a web server, I use filezilla FTP client
